We at Beacon Brook Health Center hope this email finds you well. We are excited to report that we are in receipt of the consent forms for the Covid vaccine! Please complete the attached document and email the completed document to icn@beaconbrookhc.com. Our clinic date is set for 12/28/20 and we must have the consent forms completed and returned to us by 12/24/20 to ensure CVS brings enough doses of the vaccine for all.
